Overview

An advanced clinical judgment simulation and nursing board preparation platform aligned with the NCSBN Clinical Judgment Measurement Model (NCJMM). Features 24 longitudinal unfolding patient cases, multi-tab Electronic Health Record (EHR) charts (History & Physical, Nurses' Progress Notes, Vital Signs Flows, Diagnostic Lab Panels), and a question bank of over 1,900 questions covering NCLEX-RN, HESI Specialty & A2, ATI TEAS Version 7, and Nurse Practitioner tracks.

My Role

Lead Full-Stack & Systems Engineer responsible for clinical assessment architecture, NGN decision models, and longitudinal EHR simulation interfaces.

The Problem

The Next-Generation NCLEX shifted nursing licensure from simple factual recall to multi-step clinical judgment across complex longitudinal patient scenarios. Nursing students needed authentic EHR chart interfaces and complex partial-credit scoring algorithms (+/- scoring with zero floors, rationale dyads) rather than conventional flat quizzes.

The Solution

Built a specialized clinical education web platform featuring: - 24 unfolding longitudinal patient cases modeling PACU, ICU, and Med-Surg care trajectories - Multi-tab Electronic Health Record (EHR) simulator (H&P, Nurses' Notes, Vitals flowsheets, Lab telemetry) - Algorithmic NGN partial-credit scoring (+/- scoring, 0/1 dichotomous, and rationale dyads/triads) - Bite-sized clinical sprint drills (5-question rounds, lab value flash drills) optimized for clinical shifts - Comprehensive item-level rationales detailing cellular pathophysiology and distractor contraindications

Architectural reasoning

Clinical cases require modeling progressive patient state over time. The assessment engine implements an unfolding state machine: each question step reveals subsequent clinical cues, lab results, and nursing notes based on the elapsed care timeline, mirroring real hospital EHR workflows.

Technical Decisions

  • Built an authentic multi-tab EHR chart simulator to mirror real-world hospital clinical software
  • Implemented official NCSBN NCJMM partial-credit scoring models (+/- scoring with zero floors and rationale dyads)
  • Optimized question delivery with Turbopack and lightweight SVG telemetry for zero-lag mobile shift drills

Architecture

  • Next.js App Router with Turbopack for high-speed simulation rendering
  • Longitudinal patient state machine driving unfolding 6-step NCJMM clinical cases
  • Multi-tab Electronic Health Record (EHR) interface with automated lab value abnormality flags
  • Algorithmic NGN partial-credit scoring engine with zero-floor multi-response rules
  • Telemetry and ECG rhythm visualization components for acute care case presentation

Engineering Highlights

  • Implemented 24 unfolding longitudinal clinical cases spanning post-operative, critical care, and maternal scenarios.
  • Developed item bank of 1,900+ questions covering NCLEX-RN, HESI, TEAS 7, and NP certification tracks.
  • Engineered multi-tab EHR flowsheet tracking vital trends, progress notes, and critical lab values.
  • Created shift-friendly mobile interface allowing nurses to complete 5-question drills during rotations.
  • Provided cellular pathophysiology rationales and distractor breakdowns for every question.

Results & Impact

  • Authentic NGN 6-step cognitive decision modeling with partial-credit scoring.
  • 24 unfolding longitudinal EHR patient cases with multi-tab flowsheets.
  • 1,900+ clinical questions with item-level pathophysiology rationales.
